This
Class Application is an enabled form:

you are finished, print the form, date and sign, and attach
your inoculation records

Pre-registration
is strongly recommended since classes frequently fill
Take
your completed application and inoculation records to any
Registration Sunday. Check the "Classes"
page for dates. You may also mail it to:
